Due to inclement weather cancellations, conflicting schedules, and
high demand, another FL onsite, seasonal, flu vaccination clinic has
been added. Please note newly revised schedule dates/locations.
On-Site Seasonal Flu Vaccination Clinics (revised schedule)
- Tuesday, November 3, 2009, 9:00-11:30 a.m., FL2-Café Atrium
- Wednesday, November 4, 2009, 9:00-11:30 a.m., ML-Damon
- Tuesday, November 10, 2009, 9:00-11:30 a.m., FL2-Café Atrium
- Thursday, November 12, 2009, 10:00-11:30 a.m., RAF (Jeffco)
No registration required! Open to UCAR employees, retirees, their
spouses/partners and dependent (adult) children 18-25 years of age.
For dependent children under 18, please consult with your child’s
health care provider.
Seasonal flu vaccination shots are FREE to eligible participants.
Optional nasal FluMist for the seasonal vaccine is also available upon
request at any on-site or select Passport Health locations with a
co-pay of $19 paid by participants (payable to Passport Health - cash,
checks, credit cards, and medical debit cards accepted).
VOUCHERS: Can't attend one of the on-site clinics? Vouchers are
available to get the "seasonal vaccine" at one of the following select
Passport Health’s locations. Vouchers are only valid at these
locations and are available October 1, 2009 - December 31, 2009, or
while vaccine supplies last. Appointments required at all Passport
Health locations.
