The deadline for submitting any claims for 2008 Health Care or
Dependent Care Flexible Spending is approaching. All claims have to be
submitted to PayFlex Systems no later than Tuesday, March 31, 2009. If
claims are not submitted by March 31 any remaining funds will be
forfeited. Reimbursement forms are available online in the HR Forms
Library. You may fax your claims to (402)231-4310 or mail them to
PayFlex Services, P.O. Box 3039, Omaha, NE 68103-3039.
