To schedule a blood draw appointment, visit: www.hpmi.us/ucar.html;
download the Lab Work form outlining the standard and optional (at
participant cost) blood tests available; click the appointment page
link; create a personal log in/password, choose a date/time slot and
register. The system will send email confirmations and reminder
notices prior to scheduled appointments. (Those previously registered
are still scheduled for their confirmed time slots. Reminder notices
will only be sent to those who self-enroll at www.hpmi.us/ucar.html
after today’s date.)

The standard blood screening and PSA tests are free. Optional blood
tests are available upon request and at participant cost. The list
and descriptions of available optional tests screenings and costs are
provided on the Lab Work Form. The blood draw event is open to
employees, visitors, retirees, their spouses/partners and dependent
children 18-25 years of age.

At the health fair, blood screening results can be claimed along with
a full complement of health and wellness screenings, information,
